Seven-facility practice increases care capacity with population health tool

Cedar Point Health is a seven-facility family, internal medicine, and urgent care practice in Colorado that merged several small practices in 2016 to preserve independence amid rising regulatory pressure, workforce shortages, and the need to measure quality. Preparing to join a Track 1+ ACO and shift from fee-for-service toward risk-based contracting, the organization needed tools for quality assessment and population risk stratification without adding to provider burden.

They implemented Greenway Community, a population-health platform for risk stratification, disease registries, high-utilizer identification, care management, and metric analysis, and introduced automated, “provider-independent” workflows plus care managers. The practice gained actionable data on patient risks and outcomes, increased care capacity without increasing provider workload, reduced burnout, and now confidently tracks improvements and plans custom benchmarks (for example, around opioid dependence).
