Albuquerque Health Care for the Homeless (AHCH) is a federally funded program providing primary, dental, mental health and addiction services to people experiencing homelessness. Because much of AHCH’s work happens on the street and in shelters, its old EMR — which lacked remote connectivity — forced staff to rely on paper charts and limited their ability to deliver timely, coordinated care and produce required quality and funding reports.

AHCH implemented Greenway PrimeSUITE in 2012 for its remote access and flexible templates that support medical, dental, behavioral and case‑management workflows. Real‑time access and chart updates during outreach eliminated paper charts, enabled on‑site orders and prescriptions, boosted patient engagement, and greatly simplified reporting for PCMH, grant applications and other quality initiatives, improving overall efficiency and population health outreach.
