A Greenway Health Case Study
Northwest Primary Care (NWPC) is a physician-owned, 26-provider family and internal medicine practice with eight locations in the Portland, Oregon area. Having participated in Medicare Advantage and value-sharing contracts since the 1980s, the practice faced a cultural and operational challenge: shift providers from fee-for-service habits to proactively managing patient health, lowering hospitalizations, and meeting tougher clinical and financial targets under value-based contracts.
NWPC responded by building multidisciplinary care teams (care coordinators, social workers, a high-utilizer outreach role) and managing patients in-house whenever possible, supported by Greenway’s Intergy EHR, a daily ODBC-powered reporting feed, front-and-center EHR care plans, and a patient portal for secure messaging. These changes improved care coordination and patient engagement, reduced hospital utilization, and enabled the practice to succeed in increasingly aggressive value-based contracts — from shared savings to full capitation — with sustained financial and clinical gains.
