Colonial Healthcare Improves Care, Increases Revenue through NCQA PCMH Recognition

Colonial Healthcare, a multi-site primary care practice serving Sumter, Columbia, Hartsville and Camden, SC, sought to transform care delivery and earn Patient-Centered Medical Home (PCMH) recognition. The challenge was to coordinate whole-person care across clinicians, specialists and community resources while improving access and meeting NCQA and meaningful use requirements amid a changing healthcare environment.
