Ammonoosuc Community Health Services - Customer Case Study

Ammonoosuc Community Health Services, a rural New Hampshire community health center serving more than 10,000 patients, needed a better way to deliver coordinated, patient-centered care and prove quality improvements for Patient-Centered Medical Home recognition. Using GE Healthcare’s Centricity Practice Solution electronic medical record (EMR) system, the organization sought to improve outcomes for complex patients while meeting rigorous reporting and workflow requirements.

GE Healthcare helped ACHS use its EMR data, collaboration tools, and customizable forms to support integrated care, track patient populations, and embed best practices into daily workflows. The result was Level 3 Patient-Centered Medical Home recognition, ranking among the nation’s top 26 community health centers for patient outcomes, and a pharmacy project that cut avoidable emergency department visits by 32 percent and avoidable hospitalizations by 22 percent.
