Alpena General Hospital, a 149‑bed rural regional referral center in Northeastern Michigan, was managing a paper‑heavy records environment while moving toward an electronic medical record. Facing more than 500 existing forms—including a costly 23‑part admissions form—the hospital sought a solution that could scan, index, barcode and let staff modify and map forms on demand to reduce printing, storage and workflow inefficiencies.
The hospital selected FormFast for its affordability and functionality and began a phased rollout in late 2001, starting with admissions and expanding to the Emergency Department, ICU and other units. By replacing preprinted forms with electronic, on‑demand forms they documented $62,000 in annual savings from the admissions form alone and realized additional savings in staff time, materials management and storage, faster compliance updates, and smoother cross‑department implementations.
