Columbia Valley Community Health, a Federally Qualified Health Center serving more than 24,000 patients a year in Washington’s Chelan and Douglas counties, was slowed by paper forms that delayed patient registration, clinical workflows, HR processes, and EMR data entry. The organization also struggled with chasing paperwork, inconsistent routing, and limited visibility across departments. Columbia Valley Community Health turned to Formatta for web-based forms and electronic signature capabilities.

Formatta implemented a paperless forms solution integrated with systems like NextGen EMR, ABRA HR, and Microsoft SharePoint. The result was faster registration and consent processing, better workflow visibility, and reduced liability risk with digitally signed, time-stamped records. Columbia Valley Community Health said it saved up to three and a half minutes per form, reduced overall forms printing by at least a third, and identified 289 more forms to convert, creating significant efficiency gains across the clinic.
