QTbreasthealth Works With FormAssembly Implementation Services to Enrich their Patient-Centric Healthcare Experience

QTbreasthealth worked with FormAssembly Implementation Services and consultants at Integrity Healthcare Advisors to build a compliant, user-friendly medical history form for patients scheduled for QTscan breast imaging. The company needed a way to collect medical information before appointments, connect it to Salesforce and then to its EHR system, and support the patient experience it wanted to provide.

With FormAssembly, QTbreasthealth created a Salesforce-connected, HIPAA-compliant patient information form that is emailed after a patient’s initial contact. The form takes about 5 minutes to complete and includes FormAssembly’s Chargent connector for advance payment. The company launched the form in August 2018 and said FormAssembly’s scalability was important as clinic volume grows from 8 women a day to 50 women a day by the end of the year.
