A FormAssembly Case Study
Founded in 1984, the National Gaucher Foundation supports individuals and families impacted by Gaucher Disease and manages medical assistance applications, insurance forms, patient histories, and reimbursement documentation. The nonprofit had long relied on paper-based data collection and later an in-house system that became difficult to maintain, creating administrative burdens and wasted time on data entry. Plative recommended FormAssembly to help address those issues.
With FormAssembly web forms, National Gaucher Foundation streamlined its application process into a single automated workflow that collects and inputs data into Salesforce, notifies the committee, and updates applicants. The save and resume feature made the user process simpler for applicants, and HIPAA compliance provided a secure place to upload sensitive records. The foundation reported saving more than 100 hours spent inputting patient data, with 99.9% of applicants saying FormAssembly made the user process simpler, while it serves 200 families and individuals yearly through the NGF CARE Program and gifts $2.4 million in grants yearly.
