A FormAssembly Case Study
Alosa Health, which develops academic detailing programs to share evidence-based practical information with doctors, nurses, and other healthcare practitioners, was using paper forms to collect baseline needs assessments and post-program feedback. The process created incomplete responses, hard-to-read handwriting, manual transcription into Excel, and limited visibility into whether the organization was moving the needle on program outcomes.
With FormAssembly, Alosa Health replaced paper forms with web forms for pre- and post-visit practitioner surveys, including on an iPad for field use. FormAssembly removed manual data entry, freed up office space previously used for paper storage, and made it easier to collect and transfer survey data directly into Salesforce so Alosa Health could report findings with data collected through FormAssembly.
