SEMPHN improves healthcare coordination for 1.6 million residents with FiveP

South Eastern Melbourne Primary Health Network (SEMPHN) needed to improve operational efficiency and provide its staff with robust digital systems for better information access and collaboration. To achieve a key strategic goal of organizational excellence, SEMPHN partnered with FiveP to adopt a cloud-based SharePoint intranet and optimize its use of Microsoft Teams.

FiveP implemented a two-stage solution involving a comprehensive discovery process, the deployment of the Tabei Resource Centre within Teams, and the integration of Orchestry for governance. Supported by an adoption and change management program, this provided SEMPHN with optimized, secure, and compliant collaboration tools. The project resulted in staff being able to efficiently access information from anywhere, with a clear understanding of each platform's purpose, putting SEMPHN on track to achieve its goal of fit-for-purpose efficiency.
