Faculty implemented a solution that produces a prioritised list of patients based on their hospitalisation risk. This AI-powered tool proved to be 2.6 times more accurate than clinician estimates alone. It can predict 80% of admissions up to a week in advance, enabling CERA's nurses to arrange low-cost preventative care. As a result, Faculty helped CERA halve hospital admissions, significantly reducing strain on the NHS and improving patient outcomes.
