Multispecialty Practice Launches Online Patient Health and Wellness Assessments with New User Interface

Palo Alto Medical Foundation, part of the Sutter Health network, needed a user-friendly way to launch online Health Risk and Wellness Assessments while maintaining its electronic clinical decision support (eCDS) system. Excellarate was engaged to help bridge clinical knowledge, systems, and workflow with a better user interface and support for decision-making at the point of care.

Excellarate delivered the solution using an agile approach, building modules for knowledge management, a decision engine (PREDICT), quality assurance, assessment engines for Health Risk and Wellness Assessments with action plans, administration, and reporting. Over an 18-month engagement, a team of 10 Excellarate professionals implemented the platform and continues to maintain and enhance it, helping PAMF improve clinical performance, streamline workflows, and reduce administrative effort.
