Centerstone Research Institute, a Nashville-based nonprofit focused on integrating research and technology with clinical practice, ran the coactionHealth pilot to improve engagement and care for high-utilizing Medicaid patients with complex behavioral health needs. The challenge was to give these patients a reliable, secure way to communicate with their care teams, reduce unnecessary emergency room use and inpatient stays, and better manage crises between appointments.
CRI provided smartphones preloaded with health apps—most notably HipaaBridge—to enable real-time texting, picture messaging and video check-ins with wellness coaches. The mobile program drove strong engagement, allowing coaches to support multiple patients remotely, and produced measurable outcomes: a 39% reduction in ER visits, a 53% reduction in inpatient hospital days, and significant improvements in patient–provider communication.
