Torrance Memorial Medical Center Improves Efficiency with Ensocare Solutions

Torrance Memorial Medical Center, a 446‑bed hospital in Torrance, California, faced inefficient, manual discharge-management processes that relied on printed records, phone calls and faxes, lacked reporting and visibility into post‑acute options, and made HIPAA‑compliant information sharing difficult. To address these challenges the hospital partnered with Ensocare and adopted its automated discharge solution, Transition.

Ensocare implemented a web‑based (and later Cerner EMR‑embedded) version of Transition to send standardized clinical packets and electronic referrals to a national network of post‑acute providers, reducing administrative burden and expanding referral reach. As a result, partner participation rose from 73.45% to 85.65% and provider response time improved from 1 hour to 30 minutes, while staff moved away from paper and phone workflows and gained faster, measurable visibility into discharge status.
