Magnolia Regional Health Center boosts SGLT2 fill rates 109.72% with DrFirst

Magnolia Regional Health Center, a 200-bed community hospital, faced the challenge of improving medication adherence for its congestive heart failure (CHF) patients, as low adherence leads to worse outcomes and higher hospitalization rates. To address this, they partnered with vendor DrFirst to gain access to near-real-time, clinically actionable medication history data via a population health management solution, aiming to support their new Nurse Navigator program.

The solution implemented with DrFirst provided efficient medication history data, allowing Nurse Navigators to monitor prescription fills and identify adherence gaps post-discharge through structured follow-ups. This enabled targeted patient interventions. Early results over seven months with 361 patients showed significant increases in prescription fill rates across all guideline-directed medication therapy categories, with fill rates for SGLT2 inhibitors increasing by over 109%. The program successfully accelerated the adoption of new treatment standards and improved patient compliance.
