Hospital Improves Medication Reconciliation, Streamline Workflow Using One-Click Medication History within Cerner Millennium Platform

Confluence Health, a 200‑bed health system serving north central Washington (formed by an affiliation of Wenatchee Valley Medical Center and Central Washington Hospital), faced a manual, time-consuming medication history process. Technicians relied on disparate HIS feeds, patient interviews and phone calls to pharmacies and providers, producing inconsistent, duplicate-laden reports and consuming excessive labor hours.

DrFirst’s MedHx was integrated into Confluence Health’s Cerner PowerChart/Cerner Millennium workflow, giving clinicians one‑click access to 6–12 months of medication history that is automatically mapped to the internal drug dictionary and suppresses duplicates. The result: cleaner, condensed reports (from ~10 pages to ~2), faster medication reconciliation, fewer workflow steps, measurable staff time savings and improved accuracy during admissions, transfers and discharges.
