Evolution Health, an Envision Healthcare company, needed an EMR that would let its clinician-led, post-acute and home-care teams deliver patient‑centric care while scaling rapidly through mergers and new contracts. DeVero implemented a lightweight, template‑based EMR that clinicians can use on any device, giving nurse navigators 24/7 access to cross‑enterprise records and helping prevent unnecessary ER visits and readmissions. The DeVero system supported rapid expansion—rolling out 15 agencies in two months with one‑week training—and now spans multiple sites (including 19 Evolution Health home health sites, 21 Ascension at Home health sites and 15 hospice sites) with thousands of users. As a result, DeVero helped Evolution Health increase productivity, lower total cost of ownership, and improve patient and provider satisfaction.
