Virtua Health Expands Impact of Hospital at Home with PostDischarge Pathways

Virtua Health, a five-hospital health system in southern New Jersey and the greater Philadelphia area, wanted to expand home-based care, reduce inpatient length of stay, and improve the post-hospital experience while easing pressure on hospital capacity. To support this, Virtua Health used Current Health’s remote patient monitoring and patient engagement solution as part of its Hospital at Home and Care After Discharge programs.

With Current Health, Virtua Health implemented continuous and intermittent monitoring, virtual clinician visits, medication reconciliation, and home-based support for patients transitioning out of the hospital. The programs delivered measurable results, including 2,400 bed days saved, more than 800 Hospital at Home patients treated, a 3.1-day average length of stay, a 1% readmission rate across 700+ Care After Discharge patients, a 30% reduction in unnecessary tests and consultations, an 86.5 Net Promoter Score, and the removal of 20 inpatient beds from the capital expansion plan.
