UMass Memorial Health Builds Leading Hospital at Home Program

UMass Memorial Health Care, a safety-net academic health system in Worcester, Massachusetts, faced ongoing capacity constraints and equity challenges as it worked to serve a large Medicaid and dual-eligible population. To expand care without adding physical beds, the health system launched a Hospital at Home program, using Current Health’s enterprise care-at-home platform to support monitoring, telehealth, and patient engagement in the home.

Current Health enabled UMass Memorial Health Care to build a technology-enabled Hospital at Home model with continuous and intermittent monitoring, an in-home connectivity hub, Epic integration, and Clinical Command Center support. In the first year, the program grew to a daily census of 12–16 patients, cared for more than 500 patients, and saved over 3,000 bed days, while achieving an 11.1% readmission rate that was 20–30% lower than bricks-and-mortar care.
