Imperial College Healthcare NHS Trust's Frailty Team faced significant challenges in supporting local care homes, including a lack of clinical support that led to an overreliance on emergency services and a high rate of hospital admissions for its vulnerable, frail residents. This situation was exacerbated by the COVID-19 pandemic, which made hospital transfers especially risky. The trust partnered with Current Health to address these issues.

Current Health provided a remote patient monitoring solution that included wearable sensors for residents and a platform for clinical insight. This allowed the Frailty Team to remotely monitor vital signs, conduct video assessments, and advise care home staff, enabling timely interventions. The solution led to an 87% avoidance of ED attendances, saved an estimated 144 bed days, and resulted in £62.5k in cost savings over three months for Imperial College Healthcare NHS Trust.
