They partnered with vendor Current Health to implement its remote patient monitoring platform.

Current Health provided wearables and a hub for patients to automatically transmit health data, which clinicians monitored in real-time via a mobile app. This solution allowed the Dartford and Gravesham team to intervene earlier based on data-driven alerts. The results included a 22 percent reduction in home therapy visits, a 92 percent patient adherence rate, and a decrease in emergency department readmissions, all while making patients feel safer.
