How Baptist Health improved clinical outcomes and delivered an excellent patient experience

Baptist Health, a large nonprofit health system based in Louisville, Kentucky, needed a better way to support high-risk heart failure patients at home, especially as it launched new care models and managed patients across rural and semi-rural areas with varying levels of digital comfort. Working with Current Health, Baptist Health sought an enterprise care-at-home solution that could support multiple clinical use cases and integrate with Epic.

Current Health implemented a care-at-home program with daytime vitals monitoring, nighttime alerts for higher-risk patients, daily BP and weight submissions for lower-risk patients, telehealth visits, and Clinical Command Center support. The results were strong: more than 650 patients enrolled across use cases, CHF readmissions were 0% at 30, 60, and 90 days, and 90-day survival for CHF patients reached 94%, well above the national average.
