Geisinger monitors 139 patients at home with Current Health

Geisinger faced the challenge of providing care for high-risk patients, including those with COVID-19, COPD, and heart failure, during the pandemic. To address this, they implemented a continuous in-home monitoring program using the Current Health platform.

The solution from Current Health involved shipping monitoring kits to patients' homes to provide continuous vital sign data, which was integrated into Geisinger's care workflows. This allowed care teams to proactively prioritize interventions and adjust treatments. The program achieved impressive results, including 23 averted emergency department visits and 10 avoided hospital or skilled nursing facility admissions for the 139 patients enrolled.
