Using the Current Health platform and device, Baptist Health established a remote care program that provided patients with monitoring tools and regular virtual visits. The solution enabled clinicians to adjust treatments based on daily data and check on patients proactively. This program achieved a measurable result of zero CHF-related readmissions among participating patients, empowering them to confidently manage their condition at home.
