CTG Helps Akron Children’s Hospital Adopt PCMH Care Model

Akron Children's Hospital, a major pediatric healthcare provider, faced the challenge of transforming its 28 primary care offices to the Patient-Centered Medical Home (PCMH) model within a year. This transition was necessary to move from a fee-for-service system to value-based care while maintaining revenue and addressing gaps in population health management. The hospital engaged CTG to help adopt this new care model and meet the requirements for Ohio's Comprehensive Primary Care program.

CTG implemented a strategic transformation across operations, IT, and population health management. Their solution involved restructuring clinical workflows to a team-based approach, developing new analytics dashboards and reports within the Epic system, and establishing care coordination services. As a result, all 28 offices achieved NCQA PCMH Recognition. CTG's work led to dramatic improvements, including increases in developmental screening from 20% to nearly 100% and depression screening for adolescents reaching 90%.
