Taft-Hartley Fund, a Mid-Atlantic fund serving more than 3,200 heavy equipment operators and their dependents, was seeing rising year-over-year claims, especially among high-cost claimants, along with low member engagement and weak interaction with its medical management company. Conifer Health Solutions delivered rich data analytics, quarterly reporting, predictive risk scoring through ConiferCore® technology, and personalized care management to identify at-risk members, close care gaps, and support members with new or costly conditions. The program also improved outreach through co-branded communications and nurse-authored education, producing $924,231 in savings in year 2, a 3:1 ROI, and 91% overall member engagement; disease-specific outcomes included 67% of diabetes members meeting goals and 50% of members with depression reporting better symptom control.
