Stellantis, the global automaker and mobility provider, was facing a high disease burden among plan participants, along with high emergency room utilization and low primary care use. The company was looking for a better way to help members manage chronic conditions and improve overall health outcomes, and turned to Conifer Health Solutions for care management support.

Conifer Health Solutions launched a 24-month pilot with Personal Health Nurses for employees in Indiana, Ohio, and Michigan, focusing on proactive outreach, condition management, care navigation, and education around primary care and appropriate ER use. The program was later expanded companywide after strong results, including engagement with 83% of the reached population, 96% member satisfaction, and a 2.8 ROI.
