Nationwide Company, a nationwide manufacturer with more than 100,000 blue-collar workers across multiple states, partnered with Conifer Health Solutions to pilot a healthcare program using Conifer’s Personal Health Management and care management services. The company’s challenge was that employees frequently developed chronic orthopedic issues from physically demanding work and were also overusing emergency departments and inpatient care because they were unaware of, or not using, lower-cost clinic and benefit options.

Conifer Health Solutions addressed this by deploying Personal Health Nurses (PHNs) who proactively engaged high-need members, built trust, educated employees on symptoms and care options, and coordinated access to primary care, labs, physical therapy, nutrition, diabetic classes, and same-day appointments. Conifer also added transitions-of-care support and ongoing utilization management collaboration, helping improve plan adherence and reduce avoidable ED visits and admissions. The pilot was deemed successful and expanded company-wide, with measurable savings including lower 2021 costs for engaged members in heart failure and ischemic heart disease compared with members who never engaged.
