Genesis Physicians Group needed a way to improve care for more than 25,000 Medicaid patients despite limited data science capacity and the challenge of addressing complex clinical and social needs. To support its new value-based contract arrangement and its Genovista Health population health program, Genesis turned to ClosedLoop’s healthcare data science platform to generate predictive insights from messy healthcare data.

Using ClosedLoop, Genesis rapidly built an accurate baseline model for avoidable ED visits in less than 24 hours and began using explainable risk predictions, social factor support, and trend monitoring to identify high-risk patients earlier and target interventions more effectively. The solution helped CHWs and PCPs better understand each patient’s risk drivers, connect patients to the right community-based organizations, and improve efficiency, engagement, and the potential for better health outcomes while reducing disparities.
