CiviCRM + Caldera Forms Makes Intake Easier for The POINT

The POINT is a non-profit in the Hunts Point section of the Bronx that runs after-school and summer programs for about 500 young people (grades 1–12). Their challenge was to replace a manual, fragmented intake process with a single application that captures demographic data, multiple guardian/emergency contacts, teacher/advisor contacts, special education status, and other program-specific info to support reporting and funding. They chose CiviCRM on WordPress and used Caldera Forms (with the Caldera Forms CiviCRM plugin) to build the intake.

Tadpole Collective implemented a centralized CiviCRM solution using Caldera Forms to create one dynamic intake form with conditional logic and the CiviCRM add-on to create contacts, relationships (up to ten contacts per student), and activities (program interest, equipment checkout, parental permission). The result is a streamlined, automated workflow: intake data is stored in a single CiviCRM database, reporting for grants is simplified, staff time spent on data entry is reduced, and The POINT can reliably manage intake for its ~500 participants. CiviCRM enabled the organization to standardize data capture and improve their ability to track and report outcomes.
