Memorial Hermann Southwest targets 3% of patients driving 50% of losses with The Chartis Group

Memorial Hermann Southwest (MHSW), a hospital serving a vibrant but underserved community in Southwest Houston, faced significant public health and financial challenges. These included a stark 17-year life expectancy gap between Black and white residents, a high rate of uninsurance, and a large number of potentially preventable hospitalizations. As an Anchor Institution, MHSW partnered with The Chartis Group to develop a strategic plan to address these health inequities and uplift the community.

The Chartis Group helped MHSW design an innovative, community-focused solution. This involved shifting from asset-heavy acute care to asset-light preventative care and care coordination, specifically targeting the highest utilizers of hospital services. The solution emphasized creative partnerships with other providers and a tapestry of interrelated initiatives rather than a single off-the-shelf program. As a result of this strategic planning, MHSW is now systematically executing a plan to better serve 550,000 residents, provide robust primary care to over 1,000 medically complex patients, and create hundreds of jobs through scaled workforce development initiatives.
