Lee Health, a health system in a high-growth Medicare market in Florida, faced the challenge of needing to keep pace with a projected 25% increase in Medicare admissions over 15 years without adding expensive inpatient beds. To address this, they partnered with The Chartis Group to develop and implement a hospital at home program as a key part of their strategic plan.

Working with The Chartis Group, Lee Health co-created comprehensive clinical, operating, and economic models for the program. The solution included establishing program governance and a change management plan to ensure stakeholder buy-in. The hospital at home program is projected to achieve significant results, including 20% cost savings compared to facility care, over 7,000 admissions, and more than 30,000 patient days redirected to the home within five years.
