The Hospital of the University of Pennsylvania faced increasing inpatient demand and insufficient hospital medicine provider capacity, leading to excessive emergency department backlogs. The organization partnered with The Chartis Group's Performance Transformation service to redesign its patient-centric care model and create a data-driven capacity plan.

Working with The Chartis Group, the hospital implemented an ED-based admitting team, innovative provider coverage models, and geo-cohorted teams. As a result, the hospital achieved a one-day reduction in length of stay for patients of geo-cohorted teams, a 1.6-hour reduction in average ED admit-to-provider time, and successfully rolled out the new teams to 100% of patients on the initial units.
