A Cerner Case Study
Bethesda Health Group, a long-term and post-acute care provider in St. Louis, focused on reducing resident falls at its 192‑bed Bethesda Southgate community. The care team analyzed past incidents to identify environmental and care-related contributors to falls, recognizing a need for better assessment, documentation, and communication to protect residents.
In February 2017 they implemented PowerChart® LTC, using prompted assessments, standardized PowerForms and reporting tools to guide staff, uncover hazards and standardize interventions. As a result, the community’s CMS quality measure for falls with major injury improved by 58.8%—fall rate falling from 10.14% (April 2017) to 4.17% (January 2018)—along with more accurate CMS reporting and improved team communication.
