A Cerner Case Study
Carroll County Memorial Hospital, a rural health system, faced the challenge of getting Medicare patients to engage in preventive care and annual wellness visits (AWVs) rather than only seeking care when sick. Many patients were not up to date on screenings or preventative exams, limiting early detection and management of health risks.
CCMH implemented Cerner’s Preventive Care Advisor and a dedicated Population Health Coordinator to identify eligible Medicare patients, order screenings on the spot, and conduct outreach and follow-up. The program made scheduling AWVs easier and provided ongoing support for lifestyle changes; CCMH completed 161 AWVs after launching the program in August 2016 and increased that to 256 by October 2017, improving patient engagement and early detection of health issues.
