A Cerner Case Study
KanCare, Kansas’ Medicaid program, partnered with Cerner on a Salina pilot (June 2015–July 2016) to improve the health and engagement of eligible Medicaid beneficiaries—particularly those with hypertension, diabetes, and pregnant women—who were showing low activity and at-risk biometrics. The challenge was to use limited resources to reach, engage and motivate this population to adopt healthier behaviors and connect with care.
Using targeted data to identify participants, Cerner placed services in convenient community locations, provided labs and biometric screenings, health coaching, health navigators, personalized messaging based on survey-segmented motivations, and small incentives. Participants who completed the program saw average reductions in total cholesterol (–9.8 mg/dL) and triglycerides (–27.5 mg/dL) and an increase in moderate/vigorous physical activity from 82% to 93%; the pilot’s success supports potential broader adoption.
