Peninsula Health’s shift to an EHR streamlines, improves medication administration and tracking

Peninsula Health, a multi-site health service in Victoria, Australia, faced low medication reconciliation rates and the risks of medication errors after operating with paper-based records. Their challenge was to improve medication administration, tracking and communication with primary care providers across the organization.

They implemented the Cerner Millennium EHR along with staff education and best-practice workflows, barcode patient–medication scanning, and automated discharge summaries to general practitioners. As a result, medication reconciliation within 24 hours rose from about 7–10% in 2013 to roughly 70% or higher by 2017, discharge medication lists became more complete and transparent, medication errors were reduced, and the organization earned national recognition.
