Island Health brings Hospital at Home to patients in 25 minutes with Cerner

Island Health, a healthcare provider serving Vancouver Island, faced challenges with hospital capacity and sought to provide an acute care alternative beyond traditional hospital walls. They partnered with Oracle Cerner to develop their innovative Hospital at Home program, utilizing Cerner's electronic charting system to help bring hospital-level care into patients' homes.

The solution involved implementing Cerner's technology to enable secure, real-time communication and documentation for clinicians visiting patients. This resulted in improved patient safety, experience, and outcomes, with patients often recovering faster at home. The collaboration also led to the creation of a patient-friendly medication administration record. As a result, Island Health received the 2022 Excellence in Patient Experience Award from the Canadian College of Health Leaders for the program.
