The customer, Hospice of Windsor and Essex County, faced a critical challenge when its legacy patient registration and documentation system was being sunset. This threatened to disrupt care for palliative patients, compounded by reliance on slow, error-prone paper faxes from hospitals which delayed vital treatment. To address this, the hospice partnered with vendor Cerner to implement a shared instance of the Oracle Health Millennium electronic health record (EHR) platform through the TransForm Shared Service Organization's e-VOLVE program.

The solution from Cerner involved integrating the hospice into a regional shared EHR instance used by hospital partners, enabling seamless access to patient records across care settings. This eliminated the dependency on faxes and provided a single, accessible patient history. The results significantly improved patient safety and care continuity; providers could instantly access records to prescribe medications, preventing unnecessary emergency visits, and patients no longer had to retell their stories. The collaboration with Cerner successfully created a tailored, streamlined workflow for the hospice team.
