Great River Health Systems increases annual wellness visit participation by 551 percent among Medicare patients

Great River Health Systems, a 378‑bed regional health system in West Burlington, Iowa, faced low participation in Medicare annual wellness visits (AWVs) and limited visibility into which patients were due for preventive care through their EHR.

By embedding Preventive Care Advisor into Cerner PowerChart and launching proactive outreach—calling patients, sending HRA packets, scheduling visits and entering HRA data directly into the EHR—staff could identify eligible patients in near real time and streamline workflow. From April 2016 to April 2017 Great River completed 2,389 initial AWVs (a 551% increase over the prior year), generated $1,297,651 in AWV charges, and increased providers performing AWVs from 12 to 30, boosting preventive care capture across the community.
