Portland Street Medicine (PSM), a nonprofit providing healthcare to people experiencing homelessness, faced challenges in collaboration and data management due to their highly transient client population and reliance on paper charts. They needed a modern, cloud-based tool to improve care coordination and communication. Centric Consulting was engaged to help implement a solution using Microsoft 365 and Teams.

Centric Consulting helped PSM migrate from GSuite to Microsoft 365 and implemented Teams to improve collaboration. They built user-friendly Microsoft Forms for capturing patient data, which was stored in a central cloud repository. This allowed PSM to provide more consistent care, as illustrated by a case where they effectively tracked a patient's history. The solution also enabled them to identify health trends, such as discovering that 60% of their cases involved soft-tissue issues. Centric's work provided a foundation for future innovation and earned them recognition as a Microsoft Partner of the Year finalist.
