To address this, the city partnered with Care Solace to create a central hub for care navigation.

Care Solace provided a platform and coordination services, offering residents 24/7 access to live Care Companions via phone, email, or a QR code. These companions assessed needs, found providers, secured appointments, and connected individuals to wrap-around services. Since its launch, Care Solace has supported over 10,500 residents, matching them to services in an average of 2 days and connecting them to care in 6.9 days. The solution was particularly impactful for vulnerable populations, with 71% of those assisted having public, military, or no insurance.
