West County Health Centers (WCHC) is a federally qualified health center serving about 12,000 patients in Sonoma County, California, that focuses on whole-person care by combining medical records with social, demographic, housing, crime and other public datasets. The organization struggled with fast, secure access to disparate eClinicalWorks EMR and public data sources — a process that often took three to four months for basic reports and required about 80 staff hours per month — while still meeting strict privacy, compliance and governance requirements.
WCHC implemented Boomi Data Catalog and Preparation with Tableau to automate discovery, cataloging and preparation of data across systems, applying AI to speed analysis while preserving security and compliance. As a result, reporting times dropped from months to hours or minutes, the team rapidly identified and reached at-risk patients during the 2017 wildfires by mapping EMR and NOAA data, expanded mapping to additional centers, and used self-service analytics to spot potential opioid hotspots.
