A Bonterra Apricot Case Study
United Way of Greater Mercer County (UWGMC) supports education, financial stability and health in Mercer County, NJ. Facing rising demand, tighter resources and a sector-wide shift to results-based funding, UWGMC needed to ensure funded services were coordinated, effective and accountable—especially for low-income seniors and people with disabilities who were receiving fragmented care and at risk of nursing-home placement. To address this, UWGMC convened the United Aging and Disability Partnership (UADP), a coalition of local nonprofits providing coordinated home- and community-based services under a central case-management framework.
UWGMC shifted to a fee-for-service funding model and implemented Efforts to Outcomes (ETO) software to standardize reporting, track outcomes and tie payments to reported services (with an additional 10% for overhead). The collaboration improved referral coordination, produced unduplicated client counts and gave partners actionable data to reallocate resources and attract funding. Early results show the program helps 196 at-risk clients remain at home, generating an estimated $13.66M in economic impact—saving about $69,699 per client annually by avoiding nursing-home costs—and the model is being expanded to other programs.
