Senior Community Centers of San Diego is a nonprofit that has provided meals, case management, supportive housing and wellness services to low-income, at‑risk seniors since 1970. Facing the challenge of coordinating care across programs, accurately screening hard‑to‑reach clients, and measuring outcomes to support preventative interventions, the organization needed real‑time, sharable client data to inform referrals and improve health results.

In 2013 they launched Passport to Health, deploying SoloHealth Station self‑service kiosks that upload blood pressure, BMI, heart rate and lifestyle assessments into their Efforts to Outcomes (ETO) system via a secure API. Care coordinators now use this real‑time biometric data and a built‑in risk‑assessment algorithm to make tailored referrals and track progress; the program deepens client profiles, increases routine vital monitoring, and aims to reduce ER visits and hospitalizations. A one‑year study (150 participants vs. 150 controls) is underway, building on the center’s strong program outcomes (e.g., 93.2% resolution for medical issues).
