Fairfield Family Clinic in Shreveport, LA, is a primary-care safety-net practice serving an older, chronically ill population with high walk-in demand. Facing limited onsite provider availability and the added exposure risks of COVID-19, the clinic needed a way to extend its care team, maintain chronic-care management, and protect vulnerable patients while keeping services accessible.
