Case Study: Mohawk Industries improves chronic condition care with Artemis Health

Collaborating with Clinicians for Better Chronic Condition Management

Mohawk Industries, a large self-insured employer with more than 32,000 U.S. benefits members, wanted to better connect its benefits data to its clinical resources, including on-site, near-site, and virtual clinics. To support more proactive chronic condition management, Mohawk partnered with Artemis Health and its Artemis Platform and Analytic Advisors to identify members with chronic conditions and help clinicians target care more effectively.

Artemis Health worked with Mohawk to create member cohorts across 13 common chronic conditions and build heatmap visuals showing comorbidities and risk levels. The analysis identified nearly 600 members with 5+ chronic conditions, over 3,000 with 3-4 conditions, over 12,000 with 1-2 conditions, and about 19,700 healthy members. Artemis Health then shared re-identified data with clinicians so they could focus outreach, coordinate care, and support preventive monitoring, with ongoing analysis planned over the next two years to measure impact on health outcomes and costs.
