How ConcertoHealth solved challenges providing timely and effective specialty care to their elderly and vulnerable dual-eligible patient population through leveraging the AristaMD eConsult platform

Concerto Health, a globally capitated medical group serving dual-eligible Medicare/Medicaid beneficiaries, needed a better way to connect its primary care providers with timely specialist input. The organization’s patients often faced long wait times, missed referrals, transportation barriers, and difficulty understanding specialist recommendations, which made it hard to prevent unnecessary ED visits and hospitalizations. To address this, Concerto Health turned to AristaMD and its eConsult platform.

AristaMD implemented its Smart Care eConsult platform, enabling Concerto Health PCPs to receive asynchronous specialist guidance within hours and use clinical workup checklists to improve care planning without sending patients to outside specialists. After an initial pilot, the solution was expanded across Michigan and Washington, helping Concerto Health achieve a 17.1% year-over-year drop in readmissions, a 30% reduction in hospital admissions, and more than half of eConsult responses returned in under 6 hours. The platform also replaced the need for in-person specialist visits 73% of the time and had a significant influence on the plan of care in 85% of cases.
