Improving the Patient Discharge Process at a Melbourne Hospital

Apromore worked with Leading Melbourne Hospital, a 600+ bed public hospital in Melbourne that manages more than 28,000 emergency hospitalisations a year. The hospital needed to better understand its Acute Coronary Syndrome discharge process, especially why some patients were readmitted after discharge and how to improve care while meeting the demands of rapid, efficient discharge.

Using Apromore’s process mining software, along with medical record reviews and patient interviews, the team mapped the full discharge journey and compared readmitted versus non-readmitted cases. The analysis found that non-readmitted patients typically received a nursing assessment just before discharge, while readmitted patients more often received a medical assessment; these insights were validated with ward nursing staff. The results helped identify process changes to support more person-centred discharge routines and reduce readmissions, though no specific percentage reduction was reported.
